Some common reasons the verification may fail include, but are not limited to:

There has been a recent name change. Try using the previous name.
Entering a PO Box for the address. Try entering a street address.
There has been a recent address change. Try using the previous address.
They are entering their business address instead of the signer’s home address. The signer's home address is used to confirm the identity of the signer. This is separate from the address that will be listed on the tax request for the business.
The submitted ID type does not match the user-selected ID type (e.g., driver's license vs. state ID).
ID is expired.
Photo of ID and/or selfie are blurry, glared, too dark, etc.
The live selfie does not match the ID photo.
The selfie pose position is incorrect or only their eyes move from right to left.
